Without knowing what component is causing the MISS it is hard to say.
You could try sections 88, 85, 22, 11 at level 5. All of those deal with
response cacheability in a big way.

reload-into-ims ignore-reload ignore-no-cache
Are there any recommended debug levels for use with "debug_options" that are
specific to figuring out why this happens? (besides ALL:9, which gives way too much
information...)
