It appears that moving to 3.3.5-20130607-r12573 from 3.2.11-20130524-r11822
has eliminated my problem.  I have seen a few unexplainable spikes in CPU
usage, but they haven't lasted long and squid has remained responsive.
I've been running 3.3.5-20130607-r12573 for just over two weeks without a
problem.

I haven't reproduced this in squid yet, but I recently hit a case with
similar symptoms with another threaded program on OpenBSD which hit a loop
on sched_yield if it received a signal while forking, this has now been
fixed in the thread library. So if anyone knows how to reproduce, please
try again after updating src/lib/librthread/rthread_fork.c to r1.8.
