So we conclude that there is indeed nothing in your squid.conf that 
would casue Squid to generate A and AAAA.

Perhase a "debug_options ALL,1 11,6 78,6" trace would be more informative.
If you can upgrade to a Squid-3.3 version level 44,2 will tell you 
exactly where each request is going (in 3.1 it only tells on routing 
failures).
