As the others have mentioned, the squid logs have the information
you need.

Look here: <http://www.squid-cache.org/Scripts/> for links to squid
logfile tools. I've used Calamaris
<http://cord.de/tools/squid/calamaris/> and found it to be
straightforward to use.

If you use Webmin <http://www.webmin.com/>, you'll find modules in
place to use Calamaris and Squid Logbuchauswertung (you just have to
click the icon, you don't have to spell it).
