Which version of the Berkeley DB are you using?

How are you verifying that domain3 is added but domain2 is not deleted?

What does your squidGuard.log say is happening? For example:
update: added 45 entries, deleted 16 entries

Verify that your domains.diff file does not contain any spaces or tab
characters.
