OK, let me summarize:
- squid is doing ident lookups; idents are in access.log
- squidGuard is not receiving ident information
- Not using transparent proxy mode

What version of squid are you running? Earlier versions of squid were
broken and would not send the ident unless patched. I think that was
fixed in squid 2.4.

Would you mind sending me a copy of your squid.conf file privately?
I could test some of the options that you are running to see if I
get the same results.
