I've got a pretty standard setup and download the squidGuard db on a regular basis. I just tested playboy.com and www.playboy.com, and both get blocked for me. I grep'd porn/domains and porn/urls for playboy, and only got playboy.com and a bunch of other things, but no www.playboy.com. So even without the www.playboy.com entry in the porn/domains db, squidGuard still blocks it for me based on the playboy.com entry. I know this doesn't solve your problem.
