Do you have qmail looking into the right location for the Maildir/ files? I've not used it, so I'm taking a stab in the dark. As a point, the first and last problems aren't a SquirrelMail problem, but a qmail issue. The one above you mentioned about the memory exhausted while processing the folder list, I am not entirely sure of. That sounds partially like the mail server is trying to access files, but not able to read them.
