Do you have qmail looking into the right location for the Maildir/
files? I've not used it, so I'm taking a stab in the dark. As a point,
the first and last problems aren't a SquirrelMail problem, but a qmail
issue. The one above you mentioned about the memory exhausted while
processing the folder list, I am not entirely sure of. That sounds
partially like the mail server is trying to access files, but not able
to read them. What do the logs say?

You shouldn't have to do that... that'd be a simple pain in the arse
for any ISP that'd consider qmail as a solution. Check the
documentations on allowing relaying for certain hosts.
