We've experimented the same problem:
Our environment is:

CPU:            Intel(R) XEON(TM) CPU 2.00GHz
MemTotal:       904988 kB

OS:             Red Hat Linux 7.3

With:

Qmail-ldap 1.03-20020901
Courier-imap 1.6.1
Apache 1.3.26
PHP 4.2.3
Squirrelmail 1.2.10


We've 2 servers with this configuration and the traffic on these is fairly light, but 
randomly we find one process httpd (that is serving a request on src/download.php) 
with 98-99% CPU usage for several hours.

I did't find any info on this specific problem, too.
 
Can anyone help us?
