You didn't mention what version of PHP or apache you were using. I've
seen a couple of reports of a similar problem with a certain version
of PHP, as a bug was uncovered with some configuration copies that
messed up the $_FILE variable used in attachments.

You may also want to try upgrading SM if possible as a version a while
ago (I cannot remember exactly) added an extra character on to files
that caused a similar issue.
