Sounds like your users are logging in using varying degrees of
capitalization in their usernames.  Since the addressbooks are stored as
files named username.abook, "Weasel" is very different from "weasel". 
Many IMAP servers would treat them as differently too, but some are
case-insensitive.

Look in your data directory.  If you see multiple versions of
username.abook listed, that's the problem.  There is an option in conf.pl
to always force usernames to lowercase, you may want to enable it.
