Jody,

  Not sure if you've solved this by now, but I'd guess you don't have
your permissions and/or ownership set correctly on the attachments
directory.  A typical attachments directory should look like:

drwx-wx---    2 root     apache

where apache is the group your web server runs as.  Also, when
specifying the location of that directory in conf.pl, if it is outside
of the squirrelmail directory (recommended), make sure to reference it
with the full path
