if you have installed package - according to FHS it has to be in
/etc/courier/authmysqlrc,
if you have installed it yourself - {prefix}/etc/authmysqlrc

if mysql support is not enabled - you don't have it in modules list. Your
next letter shows, that you don't have mysql support.

1. make sure, that pop3 server (qmail-pop3d + vchkpw) works
2. make sure, that you correctly compiled courier. By default courier
refuses to compile on root user, vpopmail's libraries are not available to
unprivileged user by default.
3. try loging into IMAP server without SquirrelMail. Use "telnet localhost
143".
4. check logs in /var/log
