Thanks for all the help everyone, I managed to track down the problem.  I
had been checking the apache logs but for a reason unknown even to me I
never thought to actually check the mail log!  I had entries as follows:

Error: Input/outout error
Check for proper operation and configuration of the File Access Monitor
daemon (famd).
Failed to create cache file: maildirwatch (dquinn)

A bit of googling told me i needed to install FAM so a quick "apt-get
install fam" and the folder list is nice and fast again.

Note to Self:  Read ALL of the logs!
