Did you activate the feature in your configuration file?

config/conf.pl
3.  Folder Defaults
13. Show 'Contain Sub.' Option    : true

This will display the checkbox needed when creating new folders. Remember
that different IMAP servers handles subfolders differently. If you use UW
you can't have both files and subfolders at the same time in a folder, but
it's possible in other IMAP servers depending on how mails are stored.
