could you set php error_reporting to E_ALL, display_errors to on, restart
apache and check SquirrelMail option page one more time.

If page is empty - it means php error or memory problem. Please show us
error message. If you don't provide us enough information, we can't help
you.

If you use some php cache or optimizer, make sure that you can reproduce
same error when cache is disabled.
