Which email delivery method is used? smtp or sendmail? If you use sendmail - make sure that you are using /var/qmail/bin/qmail-inject or /var/qmail/bin/sendmail and not /usr/sbin/sendmail. Or check FreeBSD sendmail wrapper configuration.
