Does not sound like a SquirrelMail problem to me.

Look in your mail log when the message goes out, see who it is being
sent from.  Then when it is at its destination, inspect ALL its
headers.

SM does not change the From or Reply-To header unless you configured
it to do so under Options->Personal Information or you have some
untrusted code running on your server.
