SquirrelMail doesn't do anything "randomly" by itself.

1) Keep your software up to date. 1.4.8 is inexcusably old.
2) Sounds to me like an unauthorized user is logging in and using a
second identity to possibly send spam.
If none of these things helps you, YOU have to find where/how to
reproduce the problem in order for anyone to help you. You might also
start watching your logs closer (install the Squirrel Logger plugin if
you need more info) to see if your accounts are being used from
unexpected locations.
