I am using Safari on the iPhone, using https, and I have cookies enabled on device. No abnormal logging in htttpd logs. Is there Squirrel Mail logs that may offer additional clues? I went back to using the original login.php and it still errors out when accessing from iPhone. Unknown user or password incorrect.
