You're sending html e-mails and doing tofu-posting, that's wrong.
My very wild guess is that change_passwd-4.0.-1.2.8 is ancient. Or I
should say obsolete.
So if you feel like updating that plugin, change line 28 to
check_plugin_setup('change_passwd', array('config.php'));
