SquirrelMail searches for default_pref file in $data_dir. If it can't find
it there, it loads file from /path/to/your-squirrelmail-install/data/
without checking.

Make sure that $data_dir setting has trailing slash. If people create config
by hand, they might miss it.

Create default_pref in /var/local/squirrelmail/data
 touch /var/local/squirrelmail/data/default_pref

Or make sure that you have standard SquirrelMail install and default data
directory is not deleted.

Please make sure that you have same problem, if data directory is owned by
apache. Give RWX access to data directory for apache user or group.
