Jason Haar (the writer of scan4virus) informed me about this:

Sure. As stated in the docs, scan4virus only scans incoming mail by default
(as it is intended for central mail servers - which don't tend to have users
actually "logging in").
Simply ensure the QMAILQUEUE environment variable is set accordingly within
the Apache (I assume you're using Apache) system startup script, and then
that variable will be around when sqwebmail calls qmail-inject - and then
it'll call $QMAILQUEUE instead of qmail-queue :-)
