when this happend for me, it was because i had my umask set to 077. you
say you've checked permissions, so maybe this isin't the problem.  
your html directory should look something like this:

%ls -al /usr/local/share/sqwebmail/html/
total 3
drwxr-xr-x  3 root  wheel   512 May 20 15:38 .
drwxr-xr-x  5 root  wheel   512 May 20 15:38 ..
lrwxr-xr-x  1 root  wheel     5 May 20 15:38 en -> en-us
