Hello, it can be some other limits set in the system, I encountered also with centos/redhat and couldn't figure out myself (well, not a sysadmin here). It is the reason I added fork_delay and modinit_delay. You have to dig in the settings of the system and try to tune them.
Happening can be somehow random, a matter of how busy the system is at that moment.
