Hello, there are several reasons why this could happen. A user agent that just disappears or has a bug in the signalization, or a bug in the Kamailio dialog module.
Hard to say without more details. If you have a SIP trace running, you can maybe find the call with the call-id (clid) from below. Then you could try to reproduce it after analysing it.
