In general, a proxy should not handle REFER - just forward it to the involved endpoints. That’s the original idea in the standards. In reality there are situations like yours where REFERs should be handled locally. How depends on the type of REFER - attended or blind. There’s always a status involved to handle the case where the party you are referring to is busy or not answering and make sure the call returns to the original state, if all parties are still around. For customers with their own PBX, I would simply just deny the REFER. For customers with phones, it’s a bit more tricky. In most cases some sort of external media bridge will be needed.
