I would recommed to test with 1.14.x. There were some performance
related improvements.

ldap_group_nesting_level = 1 would not help for id_provider AD
with default settings. Tokengroups are used by default there.

(100k+ users, 40k+ groups) is quite a lot.

What is a usual count of groups which user have?
I assume 100+
