RDMA/cxgb4: Fix memory leaks in c4iw_alloc() error paths
to the 3.15-stable tree which can be found at:

The filename of the patch is:
rdma-cxgb4-fix-memory-leaks-in-c4iw_alloc-error-paths.patch
and it can be found in the queue-3.15 subdirectory.
