Someone pinged me via IM just now about the 'code' attribute for the error element. This was retained in the schemas defined in RFC 3921 for backward compatibility, but I think it can probably be removed now (it has been almost 3 years since RFCs 3920 and 3921 were published). Also it's probably time to deprecate XEP-0086.
