Yes, I think we should require that modified versions must be clearly
marked as such. IMHO, that is an important service for readers - who
might stumble upon a modified copy of the standard and not bother to
obtain the original, simply because they assumed the copy was identical
(humans are lazy), or because they don't know where to look.
Otherwise Peter's text (see below) looks good to me.
