OK, let me rephrase that: XEP-0301 and XEP-0167 have the significant
technical difference that XEP-0301 is in-band, and XEP-0167 is out-of-band.

It's certainly possible to implement the general concept of RTT
out-of-band (as in T.140), and possible (although not very useful due to
performance problems) to implement audio/video in-band, but those aren't
the proposed standards we have for RTT and audio/video in XMPP.

I do agree with the author of XEP-0301 that sending RTT in-band has some
important advantages over out-of-band.
