Yes, I tried booting without extensions. Same result with or without loading extensions. No recent software changes.

When you booted and go the lock, did you try and hold down the shift key to disable extensions? If this worked, it might indicate a software problem. What was the last thing you did to the machine before the IDE quit? If it was a software install it could be the problem.
