We're seeing the same thing with 125100-04 on a T2000 with QLA2340
cards:

bash-3.00# uname -a
SunOS clna 5.10 Generic_125100-04 sun4v sparc SUNW,Sun-Fire-T200
bash-3.00# fcinfo hba-port -l
HBA Port WWN: 210000e08b0c8023
        OS Device Name: /dev/cfg/c2
        Manufacturer: QLogic Corp.
        Model: QLA2340
        Firmware Version: 3.3.117
FCode/BIOS Version: QLogic Fibre Channel Host Adapter fcode version
2.00.09
        Type: N-port
        State: online
        Supported Speeds: 1Gb 2Gb
        Current Speed: 2Gb
        Node WWN: 200000e08b0c8023
Error: SendRLS failed for 210000e08b0c8023

I currently have what is almost certainly the wrong FCode for qlc, but
it does the same thing with the right one, I can assure you :)
