In my current testing, I frequently destroy and recreate the iscsi targets in some different form (different disk configuration underlying each) and then have trouble having the iscsi initiator "forget" the old ones and pull in the new ones.

On the initiator >side, everything works from a fresh environment (or reboot), but after >destroying the first set of targets and creating a new set the problem occurs. Doing an "iscsadm list target" will show old targets with '-' for Alias and Connections of 0 which will remain visible in the list and the new targets will not be shown. If I do an "iscsiadm list discovery-address -v <ip>" for each of the X4500s exporting targets, the new targets are visible, but still do not show up in the "iscsiadm list target" list. The only way to reliably forget the old targets and see the new ones is to reboot the box.
