There were several related CRS, such as 6677970 (
http://bugs.opensolaris.org/view_bug.do?bug_id=6677970 ), that should
be fixed in snv_91. The general symptom of the bugs is that an
operation that should affect 1 node either affected a different node or
else multiple nodes.
Does that CR sound similar to your issue?

I just had a system blow a NIC. when I got it replaced and
rebooted, my tpgt settings appeared to have disappeared. One of them i
was able to quickly recreate. the other i ended up with tpgt 3 showing
up multiple times. after some fiddling (and restarting iscsitgtd via
smf) I ended up with the following.
host-0321# iscsitadm list tpgt -v 3
TPGT: 3
IP Address: Not set
TPGT: 3
IP Address: 10.0.0.5
Before I restarted iscsitadm, these two instances of tpgt were flipped,
and the iscsi target useing tpgt 3 was inaccessible.
Anyone know what is going on here? how do i check for it in the future?
what do i do about it? how can i prevent it? Is there some way i can
delete just the tpgt 3 with no address set.
