How on earth can the devices in the zpool and the /dev/[r]dsk's differ 
??? There is only one DVD-+rw device, why is it showing up twice?

Obviously beadm, pkg image-update, installgrub and various other things 
are breaking.

Any suggestions on how to fix would be most welcome. devfsadm and a 
reboot didn't do anything. Exporting and importing the non-root pool 
didn't change anything.

Hardware is a pretty vanilla Sun Ultra40M2, all Sun parts.

I am dazed and confused... :-)
