Sounds like an IRQ/DMA conflict(is is PCI or ISA ?) or a bad card.What I
would try in random order and one at a time :
 In BIOS set PnP to NO
             PCI IRQ sharing-no
 Recompile the modules (make modules,make modules_install)
 Reinstall the card with SAS.
--------------------------------
last resource(also in random order):
Move card to another computer-Works-yes-replace Motherboard
                                   -no -replace card
Install Linux on another partition
    works -yes-Reinstall Linux or variants to save your data
          -no -Replace card
