Guy got an old 5 1/4" game disk that had both a C64 and a IBMPC format on the *same side* of the disk; the disk was formatted by hand. I once was forced to rebuild an Apple II floppy disk directory by hand (basically an unerase the hard way), and it took me hours and hours of work.
