FormTag permits to define name and type of a form bean. 
Is seems that if bean does not exists in the specified scope, name and type are 
ignored for creation.
At line 606 of org.apache.struts.taglib.html.FormTag form bean is created by

bean = RequestUtils.createActionForm
                ((HttpServletRequest) pageContext.getRequest(),
                 mapping, appConfig, servlet);

and so i think we have loss name and type
