Form getting set null in action perform() method

I have a form that when set to the action perform() method, is null.  I have
check and double checked the following:
        - action mapping in the struts-config.xml
        - JSP file <html:form> tag
        - xForm class
        - xAction class

All of these appear to be correct and typo free.  Is there something else I
should look at?
