Do you have an Action Mapping for saveExtraPersonDetails.do? Does it use also use addExtraPersonDetailsForm? Do you have a <form-beans> <form-bean name="addExtraPersonDetailsForm" type="package.addExtraPersonDetailsForm"/>.........</form-beans> entry in struts-config.xml?
