It just means that any URL that ends with a ".do" OR starts with "/do" would be processed by the struts RequestProcessor servlet.
The "do" is insignificant, its just what all the sample applications use and has somehow stayed on. "*.do" OR "/do" Or "/struts" OR "*.struts" OR "/timbaktoo" you could use just about anything.
