Fix time of first ceiling calculation and probably 22afd4a
VPM-B profile: declare CVA iteration variables within each loop

They fix a rather rarely seen bug that isn't worth its own release (doesn't
affect planning, so won't create bad profiles). Still, it's a bug fix
rather than a new feature.
