It chooses for every trip uniformly among all possible time values in
the interval unless you give --spread.uniform which will result in
equidistant distribution of depart times.

You can either add a stop for every vehicle or a short edge with low speed.

There are several possibilities to answer this and it usually depends on
who is asking the question. From the patient / visitor side you are more
interested in waiting times and/or total travel times (maybe including
the walk from the parking lot to the entry). The hospital itself maybe
interested in different values such as access to the emergency station
or overall usage. For a general picture just activate
duration-log.statistics. For the emissions you should probably add
emissions-output for all edges using
