Your observation regarding outermost lane index is correct. I meant "rightmost in driving direction" which is equivalent for right-hand networks). If you wish to change tho origin lane for a connection you must do so in two steps: 1) remove the connection with the wrong origin lane 2) create a connection from the new origin lane to the old target lane
