I can do this as a table giving a minutes and seconds correction or as a 
graph (positive or negative).  I would recommend 3/16" thick bronze.

Either can incorporate the longitudinal correction for the planned site 
of the dial plus an inscription related to the dial/owner/location.

For a plate that size figures for five days in each month would fit and 
the rest can be interpolated by the reader.

The snags.  I can't offer a 'by return service'.  The transatlantic cost 
may deter.
If you you have no better offeres I'll cost some examples.
