This is an easy question to answer. The NASS DeltaCad macros written by the late Ron Anthony do just what you specified. They are available on the NASS website, specifically at http://www.sundials.org/links/local/deltacad/ The specific macro to to do exactly what you specified is "horizontal7.bas" but look at the others to see how features like wide gnomon and declination are added.  The source code can be read and edited in DeltaCad, any BASIC compiler or even with a text editor. The documentation is excellent so the macros are easy to change to suit your specific design. Only the usual three declination lines are built into the program. You will have to do a little editing (hacking) of the basic program for other independently selected dates and declinations. The "horizontal7.bas" macro is only 33.8 kb, almost small enough to get through the mailing list size filter. It even does the 6 o'clock style shift for the wide gnomon!
 
You do require DeltaCad. The NASS website link above tells you where to obtain a copy. You can download the free 45 day demo version that has all the features to allow you to see how effective these tools are.
