I did the snoop and there is never an inbound tftp request from the DTU.  Can I 
assume that rules out a server issue?  I know we're not filtering anything on 
the network between my DTU and the server.

That all looks sane - except that the DTU apparently failed to tftp the 
parms file. As a result it doesn't know about the newer firmware version.

As you have verified that tftp is running on the server I don't see 
anything obvious either.

You could try to snoop tftp on the server during a DTU restart to see if 
requests from the DTU come in and get a reply.
