How long did you wait after utrestart? I know sometimes it takes a minute or two for the services to come back up, and only after that does a cycled DTU work. Did you try utrestart -c as well?
There was one patch revision that I remember, either on SRSS 4.0 or 4.1 where taking one FOG member down in a clean way (i.e. init 5 or 6) temporarily disabled the other FOG members until a warm utrestart. I don't remember which revision exactly though. We haven't rolled out 4.2 in production yet, but in either event the bug was fixed in a patch quite some time ago, so if your patches are up-to-date, I don't think you will see this bug. William Yang
